Summary:

The Medical Assistant plays a vital role in supporting the medical team and patients, focusing on patient engagement, care coordination, and administrative support within a heart failure and value-based care program.

Responsibilities:

  • Engage with patients in waiting rooms to explain study details and facilitate enrollment in care management programs.
  • Coordinate patient care activities, including post-hospital discharge follow-up and remote patient monitoring, ensuring seamless communication between patients and the care team.
  • Oversee telehealth schedules and assist with telehealth visits, including patient intake, symptom assessment, and documentation of vital signs and questionnaires.
  • Maintain accurate and confidential patient records, ensuring all medical records, lab results, and insurance information are up to date prior to appointments.
  • Provide patient-centric care by assisting with medication reconciliation, coordinating lab tests, imaging, referrals, and addressing social determinants of health needs.
  • Organize and deliver patient education and information to patients and caregivers, supporting ongoing health management and care plan adherence.
  • Perform clerical and administrative duties such as processing calls, messages, and documentation, while ensuring compliance with protocols and performance metrics.
  • Participate in interdisciplinary team meetings, respond to urgent situations per protocol, and perform other duties as assigned to support optimal patient outcomes.

Qualifications:

  • Certified Medical Assistant through a recognized national certification exam (e.g., AAMA or NCCT) within one year of issuance.
  • High school diploma or GED required.
  • At least 3 months’ experience in a medical office setting preferred.
  • Strong computer skills and proficiency with electronic health records (e.g., Athena, eCW, NextGen), Microsoft Word, and Excel preferred.
  • Knowledge of medical terminology, medication procedures, and basic CPT & ICD-10 coding.
  • Excellent organizational, interpersonal, and communication skills; ability to multi-task and prioritize duties efficiently.
  • Ability to remain calm and effective in a busy environment, with a commitment to safety, attendance, punctuality, and professional appearance.
  • Goal-oriented, solutions-driven, and passionate about providing comprehensive healthcare and building relationships with patients and team members.
