Summary:
Seeking a proactive and patient-focused Medical Assistant to support a clinical team and patients, ensuring high-quality care and effective coordination within a heart failure and value-based care program.
Responsibilities:
- Engage with patients in waiting areas to explain clinical studies and facilitate enrollment in care management programs.
- Coordinate patient care activities, including post-hospital discharge follow-up and remote patient monitoring, ensuring seamless communication with the care team.
- Oversee telehealth schedules and assist with patient intake, collecting vital signs, symptoms, and relevant health information via phone.
- Maintain accurate and confidential patient records, ensuring all necessary documentation and lab results are available prior to appointments.
- Provide patient-centric care by managing medication reconciliation, coordinating lab tests, imaging, and referrals as needed.
- Deliver patient education and information to patients and caregivers, supporting ongoing health management and care plan adherence.
- Perform clerical and administrative duties, including triage, processing calls and messages, and updating insurance information.
- Participate in interdisciplinary team meetings, document work processes, and respond appropriately to urgent situations per established protocols.
Qualifications:
- Certified Medical Assistant through a recognized national certification exam (AAMA or NCCT) within one year of issuance.
- High school diploma or GED required.
- At least 3 months’ experience in a medical office setting preferred.
- Strong computer skills and proficiency with electronic health records (e.g., Athena, eCW, NextGen), Microsoft Word, and Excel preferred.
- Knowledge of medical terminology, medication procedures, and basic CPT & ICD-10 coding.
- Excellent organizational, interpersonal, and communication skills; ability to multi-task and prioritize duties efficiently.
- Ability to remain calm and effective in a busy environment, with adherence to safety, attendance, and appearance standards.
- Enjoys providing comprehensive healthcare, is goal-oriented, and committed to meeting performance metrics.
